New Patient Registration
To register with the practice please bring your medical card to reception; if your card is not available our receptionist will advise. You will then be asked to complete a new patient registration form and book an appointment for a pre-registration medical with the doctor. This gives us the opportunity to meet and to learn of your past medical history as it will take a few weeks for your medical records to reach us.
Clinical services will be provided to patients who are not on the practice list if they are staying temporarily in the area or require emergency or immediately necessary treatment, unless the patient refuses consent.
Change of Personal Details
If any of your personal details change such as address, phone number etc. please notify the practice as soon as possible, so that our records are kept up to date.
You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
